Mother’s information in the case

If the paternity or co-maternity case is processed by the Agency for Family Law, the mother has a duty to state who the child’s other parent is or may be.

On that form you may provide, as the mother, your information for the fatherhood or co-motherhood case.

The form may also be used in cases where the mother has been treated by assisted reproduction. 

How to

You must complete, sign with MitID and submit the application digitally.

You must have this information and documents ready when you complete the form:

  • Name and preferably civil registration number (CPR number) and address of the possible fathers or co-mothers.

To the extent that you have been treated by assisted reproduction, and a man must be the child’s father or a woman must be the child’s co-mother, you must enclose either 'Consent and declaration of paternity or co-maternity in assisted reproduction (Sections 27 and 27b of the Children Act)' or 'Consent and declaration of paternity or co-maternity in assisted reproduction (Section 27a, subsections 1 and 2 of the Children Act)':

Once you have completed the application and signed with MitID, the case is created at the Agency for Family Law.

After completing and submitting the application, it can take up to 24 hours before you receive a receipt in Digital Post.
