Registration of co-mother to a stillborn child
You can register the co-motherhood of your child if the child is stillborn or dies shortly after birth.
How to
Complete the form, sign with MitID and send the application digitally. You will need the other parent’s CPR number and email address so the declaration can be sent to them for signature.
You must have the following documents ready when you complete the form:
- CPR number and email address for the other parent.
- Either ‘Samtykke og erklæring om faderskab eller medmoderskab ved assisteret reproduktion (børnelovens §§ 27 og 27 b)’ [Consent and declaration of fatherhood or co-motherhood by IVF (the Children’s Act, Sections 27 and 27b)] or ‘Samtykke og erklæring om faderskab eller medmoderskab ved assisteret reproduktion (børnelovens § 27 a, stk. 1 og 2)’ [Consent and declaration of fatherhood or co-motherhood by IVF (the Children’s Act, Sections 27 a (1) and (2))]. - Samtykkerklæring (declaration of consent) (in Danish)
- If one or both of you were born before 1960 or abroad: birth and baptism certificates, or similar documentation.
- If the mother’s civil status has been changed by a foreign authority: certificate or similar documentation.
When you have completed the application and signed with MitID, the case will be registered with the Agency of Family Law (Familieretshuset).
Once you have completed and sent in the application, it may take up to 24 hrs before you receive a receipt via Digital Post.
